successful registration, log in to the EVS to complete your
registration. Ensure that your application status showing
"Awaiting Certification."

Qualifying Conditions
- Persistent pain lasting more than two weeks
- HIV or AIDS
- Alzheimer's disease
- Amyotrophic lateral sclerosis (ALS)
- Cancer
- Cachexia (extreme weight loss and muscle wasting)
- Persistent nausea not improved by traditional treatments
- Crohn’s disease or ulcerative colitis
- Epilepsy or debilitating seizures
- Multiple sclerosis or persistent, debilitating muscle spasms
- Post-traumatic stress disorder (PTSD) being treated and monitored by a licensed health therapist
- Autism
- Terminal illness with a life expectancy of less than six months
- Conditions requiring hospice care
- Rare conditions affecting fewer than 200,000 people in the U.S.
- Acute pain expected to last at least two weeks, including after surgery
